The Role: 
Are you a Social Care Worker ready to step into a specialist role that goes beyond traditional casework?

We’re building a dedicated Continuing Healthcare (CHC) Team and are looking for three passionate, skilled Social Workers to join us in shaping a forward-thinking, integrated approach to care.

This is your opportunity to work at the intersection of health and social care, applying your expertise to support individuals with complex needs through focused, short-term interventions. You’ll be part of a multi-disciplinary team, working closely with NHS colleagues to assess CHC eligibility and ensure people receive the right support at the right time.
