The Community Hospitals Team have an exciting opportunity for a Qualified Social Worker including any Newly Qualified Social Workers. We are keen to hear from you if you enjoy working in Multi -Agency Setting but have strong Social Worker ethics and values. We are a large supportive team who work with Worcestershire residents within several Urgent Care Services including Community Hospitals, Mental Health wards, Discharge to Assess placements and Out of Area Hospitals.
We work together with our Colleagues in the Acute Hospitals both in and out of County and our Health Colleagues both on the ward and in the Community.
This post is based at Princess of Wales Hospital in Bromsgrove and if you are the successful candidate, you will be working primarily with Older Adults who are patients in the Community Hospitals or in Discharge to Assess beds based in Residential Homes or Nursing Homes.
You will be working with some patients who have Complex Care Needs and as such a working knowledge and experience of Continuing Health Care is key as is Care Act Eligibility. Some of those we work with have been detained under sections of the Mental Health Act so although you won't be completing Mental Health Act Assessments you will be required to understand this work particularly Sec 117 Aftercare. You will be working with several patients who are cognitively challenged so you will also need to demonstrate you have experience of completing Assessment under the Mental Capacity Act. And be able to work withing the principles of the act.
Safeguarding and working positively with risk are part of the role as well as understanding the need to support Carers and Families at what can be a stressful overwhelming time.
While we are keen to minimise unnecessary road journeys we do at times have referrals from our colleagues in Hereford and would expect you to be able to travel there to carry out assessments if required.

We have several Practice Educators within the Team so Social Workers have plenty of opportunity to share their knowledge and learning and we pride ourselves on the support and camaraderie the Team provides. The Team meets regularly in Team Meetings where we have guest speakers and in smaller cluster meetings with ASWP’s where workers have an opportunity to share aspects of good practice or share some of the challenges. You will have regular Supervision and peer support to ensure you are able to continue your career development and career aspirations.
If you are a Newly Qualified Social Worker embarking on your ASYE we will offer support from one of the very experienced PE in partnership with our Social Work Academy in line with the Local Authority ASYE Guidance.
You will be expected to be ward based when assessing patients as these will need to be face to face, although there is flexibility for you to be able to work from home on agreed days
