Are you passionate about making a difference to the lives of adults with complex health and social care needs? We are seeking a dedicated, compassionate, and motivated Social Care Worker to join our Continuing Healthcare (CHC) Team.

This is an exciting opportunity to work within a multi-disciplinary environment, supporting individuals and their families through assessment, care planning, and review processes whilst promoting wellbeing, independence, and person-centred outcomes.

About the Role

As a Social Care Worker within the CHC Team, you will:

  • Support the assessment and review of individuals with complex health and social care needs.
  • Work closely with NHS colleagues, care providers, service users, families, and carers.
  • Assist in gathering information to contribute to Continuing Healthcare assessments and decision-making processes.
  • Promote person-centred, strengths-based approaches to care and support planning.
  • Monitor care packages and ensure services are meeting agreed outcomes.
  • Maintain accurate records and documentation in line with organisational and legislative requirements.
  • Support safeguarding processes and contribute to risk assessments where appropriate.
  • Help individuals navigate health and social care systems to access the support they require.

About You

We are looking for someone who:

  • Has experience working within health, social care, or a related field.
  • Demonstrates excellent communication and interpersonal skills.
  • Is committed to delivering high-quality, person-centred care.
  • Can work effectively as part of a multi-disciplinary team.
  • Has strong organisational and record-keeping skills.
  • Understands the principles of safeguarding, the Care Act, and person-centred practice.
  • Can manage competing priorities and work independently when required.
