Salary: £34811 - £47665 per annum with additional £3000 market forces
37 Hours per Week

The Role:
We are seeking an enthusiastic and experienced social worker to join our Continuing Healthcare (CHC) Social Work Team in a unique split role, spending 50% of your time working as an Advanced Social Work Practitioner and 50% as a Social Worker.
This exciting opportunity combines direct practice with leadership responsibilities, allowing you to maintain a caseload while supporting excellent social work practice across the team. You will work collaboratively with health colleagues, individuals, families, and carers to deliver person-centred, strengths-based interventions and contribute to high-quality CHC assessments and reviews.
What You'll Be Doing
Advanced Social Work Practitioner (50%)
In your Advanced Practitioner role, you will:
- Provide professional leadership and practice expertise within the CHC Team.
- Offer consultation, coaching and mentoring to social workers and other team members.
- Support complex case discussions, risk management and decision-making..
- Contribute to service development, quality assurance and practice improvement initiatives.
- Support the implementation of relevant legislation, policy and best practice.
- Assist with the induction and development of newly qualified and experienced staff.
Social Worker (50%)
In your Social Worker role, you will:
- Manage a complex caseload within the CHC service.
- Complete Care Act assessments, reviews and support planning.
- Undertake CHC Checklists, Decision Support Tool assessments and joint working with NHS colleagues.
- Work in partnership with individuals, carers and multidisciplinary professionals..
- Promote independence, wellbeing and positive outcomes for adults with care and support needs.
Essential Requirements
- Qualified Social Worker with Social Work England registration.
- Significant post-qualifying experience within Adult Social Care.
- Experience of working with adults with complex needs.
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act and safeguarding legislation.
- Experience of multidisciplinary working with health partners.
- Ability to provide professional support and guidance to colleagues.
- Excellent assessment, analytical and communication skills.
