Worcestershire County Council is committed to supporting its residents to live healthy and independent lives in their community. In order to achieve this, the Council, in partnership with NHS colleagues and our care providers, provide a Reablement service that will assist adults referred to the council for social care to regain their independence.

Our approach will be to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible. 

We are looking to recruit a permanent Co-ordinator who will be responsible for planning the programs of work for Reablement Assistants using our IT systems. As the ideal candidate you must have experience of multi-tasking, working to tight deadlines and being able to think on your feet. The reablement service is fast-paced and constantly changing so therefore you need to be able to react quickly and efficiently. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate, and empathetic. 

The service is provided from 07.00 to 22.00. It is centrally managed through a Call Centre based in County Hall, Worcester. The post holder would take part in the 8-week rolling rota which covers 7 day working (including weekends and Bank Holidays). The role is a combination of being office based and home working.
