Job Title: Relief Service Assessor 
Salary: £15.84 - £16.93 per hour, plus 14.49% enhancement
0 Hours per Week
Location: Countywide

We will continue to focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible. This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life, and remain independent for as long as possible. We are looking to recruit to the position of Service Assessor, you must be committed to:


    Undertake initial assessments within the community and countywide, identifying needs, risks, and the service user's potential for regaining independence with personal and / or domestic activities of daily living.

    Develop Promoting Independence Plans that capture the desired service user outcomes and detail the required intervention of the service.    

 •    Actively complete regular reviews of their progress, to support with flow and capacity within the service.

 •    Ensure that all standards and performance measures are adhered to, including service user monitoring protocols and quality assurance audit completion

. •    Ability to be able to confidently explain processes

. •    Excellent recording skills on IT systems. As the ideal candidate you must have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate, and empathetic.

Disclosure Barring Service (DBS)
This post is exempt under the Rehabilitation of Offenders Act 1974 and as such appointment will be conditional upon the receipt of a satisfactory response to a check of criminal records via the Disclosure and Barring Service before the appointment is confirmed.
